What You'll Need before You Start

  • A Witness

    - The witness can be a family member, a friend, a band staff, etc.
    - The witness will be required to sign the application.

  • Your Unique Email Address

    - Must be your own personal email address and not borrowed from someone else.

    - If you don't have an email address, please visit a PBCN band office for assistance, or click here for instructions on how to create a Gmail.

  • A picture or PDF copy of your Identification Document (ID)
    - The ID should include your photo (e.g., Treaty Card, Driver’s License, SGI Card, Passport, etc.).

  • Your Certificate of Indian Status Number (status card/treaty card)

  • A picture or PDF copy of your Void Cheque/Direct Deposit Slip
    - The Void Cheque or Direct Deposit Slip must be from your personal bank account.
    - Must include your full legal name and address as provided above

    - Must be obtained directly from your bank’s official website, mobile application, or branch. Documents from third-party sources will not be accepted.

    - This document will be subject to verification by our staff.

  • Details of your Bank Information

    - For Canadian Bank Accounts: Bank name, bank number, transit number, account holder's name, account number, and a copy of your Void Cheque/Direct Deposit Slip.

    - For U.S./International Bank Accounts: Bank name, bank address, Swift code, account holder's name, account number, and a copy of your Wire Transfer Instruction Document.

‼️Important‼️

  1. Must use 01 email address per applicant (your email address should not be used for multiple applicants, your email address should not be used for a family member).

  2. Each applicant must use their own—unique email address.

  3. Do not use the same email address for multiple family members. If the same email address is used for more than one application, it will be treated as an update, and the new information will overwrite the previous submission(s).

  4. If this has occurred, please reapply with a different email address for each applicant.
